When the pilot teams are running FMEA comfortably within their sprints, weeks 5-6 shift focus to rolling the process out across the organization and making it self-sustaining. This includes standardizing scoring criteria so risk data is comparable across teams, wiring risk gates into CI/CD pipelines, and establishing quarterly review cycles that keep the methodology sharp as your applications and architecture evolve.

Week 5: Organization rollout
